Everyone requires blood; it is needed to aid in healing and to sustain life. You or someone you know may have needed blood at one time. Without blood donations, life saving procedures and research could be postponed or even cancelled.

You realize the importance of blood being available and ready at a moments notice. You can play a vital role in the lives of others by making certain there is an adequate supply of blood by donating regularly.

The Blood 4 Life Donor Program offers something for everyone. You can earn merchandise rewards in appreciation for your participation.

What is the Blood 4 Life Donor Program ?

The Blood 4 Life Donor Program is a point appreciation program to say “Thanks” for donating blood frequently.

Each time you donate blood at Blood 4 Life Centers, participants receive Blood Donor Points, which can be redeemed at        ,our on-line store. There are 4 different redemption levels, each offering over 30 items to choose from. You can select an item for yourself or have it sent to a loved one on your behalf.

• Plasma
• Double Red Cell
• Plasma/Platelets
• Platelets

Points are issued at the end of each month. Point statements are available to each participant at        or you can contact the Program Administrator at ____________________________.

How do I join Blood 4 Life ?

• Complete a Blood 4 Life Registration Card
• Cards are available at the Blood Center or click here.
• Register today and earn 1000 Bonus Blood Donor Points.

• Donate blood at least once per quarter.
• Blood is needed year round; if everyone donated one time each calendar quarter, we would never have a blood shortage.

• Allow us to contact you.
• We respect your privacy and want to communicate via your preferred method.

• Spread the Word
• Share your experience with others; invite them to join you in making a difference in the lives of others.
  
  

How Do I Redeem my Blood Donor Points?

There are two ways to order:

Points

• Points may be directed back to the Blood Center.
• Your points cannot be donated to other participants.
• Your points cannot be shared with another person’s points in order to redeem a gift.
• Your points are valuable to you and we want you to use them for you and your family.
• Points have no cash value.
